DS-160 is the mandatory online application form for USA visa application. It includes detailed questions such as personal information, travel history, work and education information. The form must be filled in English and every information must be correct and consistent.
In case of visa rejection, try to understand the reason for rejection and address the deficiencies. You can reapply with new documents or changed circumstances. There is no appeal mechanism for rejection decision.
With B1/B2 visa, usually up to 6 months stay right is granted, but this time is determined by the officer at the time of entry. Visa validity period (usually 10 years) and stay time are different concepts.
